Athletic Packet Forms

All student athletes must complete these forms prior to starting practice.

In order to participate in sports at John Carroll Catholic High School, all students must comply and complete the following

• Must maintain a 2.0 Grade Point Average on a 4.0 Scale
  (Incoming 9th graders are grand fathered in)
• Be cleared by a Medical Doctor or Doctor of Osteopathy and complete the
  FHSAA Preparticipation Physical Evaluation
• Complete FHSAA Consent and Release from Liability Certificate
• Complete and get notarized the JCCHS Travel/Insurance Form
• Pay $100 Athletic Fee
