Welcome to John Carroll High School!

Prospective 9th graders should follow the registration / application process as listed:

1. Print out and complete the following forms:

"Prospective Student Admission Application",
"Student Information Request Form"
"Parish Affiliation Form" (if applicable)

Once the forms are completed, mail them in or drop them off with a testing fee payment of $30.00. Make checks payable to John Carroll High School or pay online to fulfill the first step in the prospective 9th grade registration.

Upon receipt of your forms and payment, the JCHS Guidance Office will contact you to confirm your seat for the next placement test, which is a requirement for all 9th grade applicants.

2. Placement Test

All prospective 9th graders must take the placement exam, which is offered the first Saturday in December, again in March, and as needed for late applicants. We encourage all interested students to test sooner than later as tested students are reviewed first for acceptance.

The placement exam is used for placement into one of our three academic levels of college preparatory curriculum. If a student does not score well on the placement exam for the desired level of placement, the exam may be taken again at their request and expense. The new results will be reviewed, and if necessary a new academic placement will be determined.

3. Complete “Tuition Contract Form”

Following acceptance, parents must complete the “Tuition Payment Preference and Tuition Contract Form” through the Business Manager’s Office. This form is also available on this link.

Copy of Birth Certificate
Copy of Social Security Card
Copy of Immunization Records (HRS 680)

If applicable: Copy of Custodial Agreement
